A child in the Pre K classroom had a tablet watching videos that were not related to the lesson plan. NOTE: This was corrected at inspection when the caregiver removed the tablet and put it away.

Staff files were reviewed the staff files were missing the Pre-Employment Affidavit.

One out of five employee records reviewed did not have the required trainings emergency preparedness, building safety, and allergic reactions for the last full year of employment.

Checked three caregiver records plus director records. Director records is missing one hour of child abuse and neglect training for the annual year. One out of the three caregivers is missing one hour of child abuse and neglect training for their annual year.

A cockroach was seen in the after school classroom.

There was food stored in the refrigerator without a lid. NOTE: This was corrected at inspection when food was taken out of the refrigerator and disposed of properly in the garbage bin.

It was observed during the inspection the most recent fire inspection was posted.

The recent fire inspection was not available for review.

The food menu was not posted in the operation. Corrected at Inspection: The menu was posted during the inspection.
