The designee and director of the program was made aware of the allegations and concerns on 11/13/25, but a report wasn't called in to report these allegations until 7 days later, outside of the required reporting requirements. By the time of the inspection at the facility, a plan had been put in place to ensure that all allegations and/or concerns are reported in a timely manner to Licensing so that investigative measures can be taken.
