A 3 month old infant was observed sleeping in a swing (restrictive device), and was not moved into a crib until inspector informed caregiver. Note: The infant was removed and placed in its individual crib.
During a follow up Inspection, it was observed that in the playground area the surfacing material (loose fill) was not 6 inches but rather 2.5 inches.
A diaper changing mat in the Infant classroom was observed torn.
