Observed that air conditioning was not working in bus #2.

Observed that changing mat was not sanitized after each diaper change.

Observed a child's hands were not washed after a diaper change.

Observed a caregiver changing a diaper and caregiver did not wash hands afterwards.

Observed a caregiver stepped away briefly to the other side of the classroom from changing table while a child remain strapped during a diaper change.

Severe weather drills and lock down drills have not been documented.

A caregiver left a group of seven children unsupervised briefly. NOTE: Caregiver stepped back into classroom.

A fire drill has not been practiced.

Observed that three children files are missing proof of immunization.

This standard was found deficient as part of a DFPS Investigation.

Based on DFPS findings, a caregiver pushed and hit a child. Caregiver picked up a child and threw child on a mat.

Based on DFPS findings, a child was held down by a caregiver to force child to rest or sleep.
