During the investigation, it was determined that staff did not intervene in a timely manner when a child was injured in care.
It was observed one staff member had two infants that are 13 months old, and a 4 year old child in the main room, more than 18 months apart. Based of the ages staff member was out of ratio.
It was observed that one infant sitting in a high chair did not have safety straps buckled.
During the inspection ten children's files were audited, and four children's files did not have updated immunization records.
A Food Allergy & Anaphylaxis Care Plan (FARE) did not have a physician's signature.
