A 15 month old in the infant room was observed unbuckled while in the kidney bean shaped table. This was corrected at inspection when the infant was buckled in bringing the operation into compliance.

The loose fill on the playground under the slide was not maintained to 6 inches.

It was determined throughout the course of the HHS Investigation that a young child in care was left alone on the playground unsupervised for approximately 7 minutes.

It was determined throughout the course of the HHS Investigation that parent was not contacted immediately when a child was placed at risk. The parent was contacted approximately 3 hours post-incident.

The toilet between the toddler room and the preschool room was not flushing correctly. The plumber was called during this inspection.
