A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
A newly hired employee with unsupervised access to children did not have current first aid.
One caregiver who started on 9/9/24 did not have a pre-employment affidavit on file. This was corrected when the form was completed, signed and added to the file.
Cotton balls were missing from the first aid kit. NOTE: The deficiency was corrected at inspection.
The operational policies are lacking a physical activity policy.
Annual training in SIDS, Shaken Baby Syndrome, early childhood brain development, abuse/neglect and special topics were not made available for review for the director.
Loose fill surfacing measures less an inch for a peice of rotating equipment. Loose-fill surfacing is lacking for another piece of climbing equipment with an attached slide.
The posted activity plans do not include outdoor activities once per day for infants or twice per day for older children.
Current liability insurance was not made available for review.
