A caregiver was yelling at students by calling them names and speaking in a high tone pitch.

There was an infant observed for a minimum of five minutes sleep in a restrictive device. Caregiver was asked to remove infant during inspection.

During today's inspection, there were 8 children, ages 12-19m, in a classroom with one caregiver during nap time.

Two caregivers did not have signed documentation showing they received the operational policies.

Two caregivers did not have all the required 24 hours of training for last training year. Three caregivers did not have the required topics of training.

While the child had a crib in the room, a 10 month old non-walking infant was observed on a cot with blankets for naptime.

This standard was part of a DFPS investigation. It was determined that a caregiver "smacked" a child's hand as a form of discipline.

This standard was part of an investigation. It was determined that the caregiver in question did not have completed pre-service or orientation documents on file.

Children were observed sitting in a table chair without straps. This standard was corrected during the inspection, straps were added to the seats.

The carbon monoxide detector does not get tested monthly.
