The operations fire inspection has last been conducted on 7/22/2025, making the inspection expired.

Staff files were reviewed 3 out of the 7 staff files that were reviewer were missing training in the following topics, Understanding building and physical premises safety and handling, storing and disposing of hazardous materials.

Annual Fire Inspection is not up to date. Last annual fire inspection was on 8/23/2022.

Toddlers were being change in a changing pad that was above the floor level, the changing pad did not have a safety mechanism such as a safety rail or raised sides. NOTE: This was corrected at inspection when caregiver placed the changing pad on the floor when changing the children.

The first aid kit was reviewed and antiseptic wipes were not in the first aid kit.

In the playground there were 3 toys that was broken and children could not use it properly making it a hazard for the children. NOTE: This was corrected at inspection when caregiver removed the toys from the outdoor play area.

There is a TV in the toddler room that is not anchored.

One of the staff members that drives and participates during transportation route does not have transportation training.

Eletronic Child Safety Alarm is currently not working.

Driver's License for one of the transportation drivers was not available for review at the time of inspection.

Front door is labeled as an emergency exit and was locked upon arrival. A staff member was not available to open/unlocked door for approximately five minutes.
