My Preschool & Child Development Center - South

There were observed to be several choking hazards in three different center play areas accessible to children from 18 months to 5 years old.

In the outside play area a four level shelf was observed to be unstable and unmounted which poses a tipping or fall hazard. It is accessible to children during active play.

A child was found outside alone and unsupervised for approximately three minutes.

A parent was not notified immediately after, when their child was left alone outside unsupervised.

A child was found unsupervised in a classroom for approximately two and a half minutes.

Inspector observed two infants with blankets while in care. One infant was 2 months and the other was 6 months old. Inspector also observed the 6 month old a second time with a blanket with a different caregiver in the room.

The director did not have documentation of 30hrs of annual training as required.
