One staff background check was still active in the system. Based on the information obtained, the last day of employment for staff was 08/05/2026.

A choking incident occurred when a child was served and consumed a food item that posed as a risk, which resulted in a choking hazard.

The most recent fire inspection was dated, 04/15/2025.

Two staff members observed caring for children were not aware of the ages of the children, in which they were responsible for.

Two children's file observed did not have a health statement available for review

The transportation safety alarm was not in working condition.

One child's file did not have an immunization record available for review.

A total of 18 children, 3-5 years of age was observed in the Preschool 3 classroom with one caregiver. The specified age group is 3 years of age. A total of 20 children were observed in the Primary 1 classroom, 3-5 years of age with one caregiver. The specified age group is 4 years of age. A total of 20 children, 19 months - 36 months of age were observed in the Transitional 1 classroom with two caregivers.

Caregivers did not complete the required training necessary to care for a child, and/or children with special needs.

The nap mats were not labeled in the Transitional 2 and Toddlers classroom.

Good judgment was not utilized when a child was posted on social media without the parent's authorization, and/or consent. Based on the video footage observed, a child was posted on the operation's social media account.

The emergency practice form observed did not include the time of drill, the length of the drill conducted, the length time of time for evacuation, sheltering, and/or the lock down drill.

The most recent fire inspection was dated, 11/21/2023.

Two staff files observed did not have the training certificates, and/or records available for review in order for licensing staff to properly evaluate staff training.

A recent annual sanitation inspection was not available for review. The most recent sanitation inspection was dated, 09/14/2023.

The childcare operation has not inspected their fire extinguishers since September of 2024. The fire extinguishers have not been serviced since September of 2023.

Based on the information obtained, parent's rights were not provided to parents within the required timeframe, by 09/30/2023. The parent's rights information was not updated within the childcare operational policies. The childcare operation did not have a receipt/acknowledgement from parent's stating that they received a copy of the parent's right. Children enrollment forms were not updated to portray receipt of acknowledgement of parent's rights.

One toilet in the Primary 2 classroom did not work, and/or was not accessible to children in care.
