Child was left unsupervised for approximately four minutes.

Transition sheets were not completed at the end of the day.

Operation was missing Maintenace checklists.

Bus 2 was missing scissors. This was corrected at inspection when the director added scissors to the first aid kit.

The test data for surfacing materials was not available for review during the inspection.

A fire drill was not conducted for the month of April. Documentation was not available for review at the time of the inspection.

A current fire inspection was not available for review at the time of the inspection.

I observed a food allergy emergency plan form that is not signed by a doctor.

Based on information gathered during the investigation, appropriate supervision with not provided, resulting in one child being bitten and another child being injured.

Based on the investigation it was determined that a caregiver used inappropriate discipline on a child. This was corrected at inspection as the caregiver was terminated. Inappropriate discipline was not observed at inspection.

A child was left unsupervised in the building while the class was outdoors.
