Based on the DFPS investigation, it was found that caregivers were using their cell phones while providing care to children.
Per the DFPS investigation, it was determined that the caregiver was not adequately supervising the children on the playground when a child inappropriately touched another child in care.
The operation did not report the incident involving a child who was placed at risk. The site director was alerted reagarding the incident Jan 28th via email.
The two caregivers present did not know how many children they were responsible for. This was corrected when the attendance sheet was completed during dinner.
One caregiver was supervising 44 students in the cafeteria while the other teacher took a group of children to the restroom.
Director changes were not reported to Licensing.
Observed 64 students in the cafeteria with two caregivers. The children were running around the cafeteria, opening the door for visitors, throwing food and chocolate milk cartons at each other, screaming/yelling and tusseling with each other, and jumping off the stage.
A staff member supervising a group of children has a pending background check on file.
