A child in care was left unsupervised in a classroom.
One employee was observed working with children in care and did not have a completed background check with the operation. This was corrected when the employee was removed from caring for children and a background check was submitted to be completed.
A child was left inside a classroom alone to use the restroom.
During the investigation it was found that a child had been left sleeping in a classroom without a caregiver for between 2-4 minutes. This was corrected at inspection when supervision was observed to meet compliance and the caregiver responsible for this incident was found to no longer be employed with the operation.
It was reported and found that 3 infants were left unsupervised in a classroom when the caregiver had left the room.
Based on the information and documentation obtained, parental communication regarding three infants being left unsupervised did not occur immediately after ensuring the safety of the children.
The on-line CPR recertification training obtain did not include use of a CPR manikin.

Video footage was received of the child being left alone by her caregiver. The former caregiver also admitted that the child was left behind when entering back the building.
Based on information obtained through investigation interviews, there is sufficient evidence to suggest that the operation did not inform licensing of the incident that occurred.
Based on the information obtained during the investigation it is determined that the operation did not report an injury that required a child to seek medical attention.
Three out of ten children files evaluated need an updated immunization record.
