It was found that a caregiver did not have an initial background check submitted on 4/15/24 when caring for toddler children during an incident when a child injured themselves requiring medical treatment.

Two classrooms were found to have paint damaged. The crown molding in the kindergarten classroom and chipped paint in the Younger 3 classroom.
While reviewing staff records, it was discovered that an employee did not have an attendance or timesheet available for review.
I evaluated 10 children's records. I observed the following information missing: 1. One child was missing parent permissions for fieldtrips, transportation, and water activities. 2. One child was missing hearing and vision screening results and a pre-school health statement. 3. One child needed contact information for the physician.
