During the investigation, interviews and other evidence collected determined that the operation was found deficient with this Minimum Standard. The evidence established that the operation failed to pick up a child from school and transport the child back to the operation as scheduled.

The evidence established that, although the parent notified the operation that the child required transportation from school, the child was not included on the transportation list and was not picked up as scheduled. As a result, the operation failed to maintain and utilize an accurate list of children to be transported, resulting in the child being left at school.
During an investigation it was found that a caregiver placed tape over a child's mouth for a few seconds. This was corrected when the tape was removed.
During interviews it was found that a previous caregiver utilized prohibited punishment on a child in response to the child hitting at her face.
A caregiver was observed alone in a classroom with 14 children aged 2 years old. This was corrected when the second caregiver returned to the classroom.
A caregiver reacted to children in care, by grabbing their faces to get them to listen.
A caregiver did not use self-control when disciplining a child. the caregiver grabbed children by the face in an aggressive manner to gain their attention.

First aid kit supplies wa expired. This was corrected during the inspection when the items were removed and discarded.
One caregiver stated that they had 6 children in care but there were 7 children in the caregivers room. I explained to the caregiver that they must always know how many children that they have.
An allegation was brought up to staff about suspected prohibited punishments being used and this was not reported as required. I spoke with the staff about how to make a report and gave them the intake phone number for future reports.
This standard was found deficient as part of a DFPS investigation.
During a DFPS investigation, it was found that a child in care was served a food that the child is allergic to.
