During an investigation inspection it was found a child was left unattended for a brief moment.

Upon reviewing CPR/First Aid training records for caregivers, as part of the investigation, it was determined that one caregiver did not have proper training for CPR. The training was completed online only and did not include the required hands-on skills component.

The annual fire inspection was not available for review at the time of the inspection.

Three classrooms did not have evacuation plans posted.

The operation did not have a current sanitation inspectin on file.

It was found during the inspection caregivers were missing trainings in required annual topics.

It was found during the inspection the operation did not have a current fire inspection.

It was found during the investigation the operation left a child in the hallway unsupervised.

It was found during the inspection a caregiver was present without a background check. This was corrected when the caregiver left the operation.

It was found during the investigation a caregiver left a child on the playground unsupervised.

It was found during the investigation, the operation did not contact a child's parents immediately after the child was left on the playground unsupervised.

One child was napping behind the changing table out of sight from the caregiver in the room. This was corrected at inspection when the caregiver moved the child from behind the changing table.

One of the eight rooms during nap time did not have adequate lighting for supervision. This was corrected at inspection when the caregiver turned on the dimmed lights.

Out of ten personnel files reviewed: - two personnel files were missing 9.5 hours of annual training - one personnel file was missing 11.5 hours of annual training.

The operation had two bottles of expired medication.
