During the observation of the video, it was observed the caregiver holding a child by the forearm and walking to the carpet. The caregiver pulled the child to the floor by one arm, resulting in the child receiving an injury of Nursemaids Elbow.
A caregiver was observed on the video footage grabbing children by one arm and placing them down.
After reviewing the video footage is determined prohibited punishment did occur. Caregiver used the shoe to hit a child on the foot.
The operation had a child in care with a food allergy to eggs and no signed Food Allergy Emergency Plan signed by the parent or the physician.
The operation had medication (Epi Pen) for a child that was not in the original packaging, with the child's name on it. Note: This was corrected at inspection when the operations, assistant director gave the medication in a bag to the parent when they parent came to pick the child up.
