Pre-K Classroom was observed having screen time during the snack meal.

The sanitation inspection report was found expired with the date of 4/14/25 and the current inspection report not available for review. The documentation does not show that a inspection has been conducted within the last 12 months.

Based on the evidence obtained during the investigation, ratios for two-year-olds were not maintained during the field trip.

Based on information obtained through the investigation, Caregivers were not easily identifiable by all children attending the field trip.

There was no documentation of monthly smoke detector test during inspection. Multiple smoke detectors were heard chirping during the inspection.

Fire drills were observed not documented for the months of February and April during the inspection.

The was exposed sheetrock and a hole in the wall in the 18-23 month classroom.

There were two mats being used to sleep on in the PreK classroom that had rips on them. This was corrected when the caregiver change mats to new mats.

The most recent fire inspection has expired.

The infant room had an uncovered trashcan with soiled diapers in the can. The diapers were not in individual sealed in bags. The can was emptied during the inspection.

The operation has standing fans in each classroom. The fans can be a safety hazard when not mounted out of the children's reach or without safegaurds.
