A hole was observed in the wall of the bathroom. The fence on the playground is not in good repair. The hole was repaired during the inspection when the caregiver patched it.

2 of the 5 children records reviewed did not have health statements.

The fire extinguishers have not been serviced since August of 2023.

2 of the 5 children records reviewed were missing immunization records.

There was no documentation of drills conducted for the year 2024.

The fire inspection has not been conducted. The last fire inspection was conducted on 08/25/23.

The loose fill underneath the slide was not at a depth of at least six inches.

Out of six personnel records reviewed, the following was observed: Six records were missing complete training certificates; Two records were missing notarized affidavits; Two records were missing photo identification; One record was missing proof of background check eligibility results; and, One record was missing proof of education.

Out of six children's records reviewed, the following was observed: Four records were missing a health statement; Three records were missing complete immunizations; Three records were missing emergency contact addresses; One record was missing hearing and vision; and, One record was missing physician information.

Two classrooms were observed out of ratio.

One child was observed walking from one room to another room unsupervised. There was no line of sight between the classrooms.
