An infant was observed seated in a highchair and the safety straps were not being utilized. Noted: This was corrected immediately when the safety straps were fastened.

A caregiver was observed diapering a child utilizing gloves. Once completed the gloves were removed and the caregivers hands were not washed before applying new pair of gloves and completed another child's change. Noted: this was correct immediately when the caregiver was instructed to wash their hands.

A caregiver gave incorrect number of children she was responsible for during naptime. Caregiver had eleven sleeping children in her class, yet stated she had ten.

A staff member was observed caring for children without a cleared background check.

An incident report was not created for an incident that placed a child at risk. The child was left outside for approximately 3-5 minutes unattended.

The operation failed to notify CCR that a child was left outside alone within the required 48 hour timeframe.

Caregivers left a child outside alone and were unaware of his whereabouts for approximately 3-5 minutes

Two year old classroom was found out of ratio. The classroom had 13 children for one teacher. Note: Two children were moved to another classroom immediately.

The ceiling in the 3 year old room and both 4 year old bathroom have broken ceiling tiles. Corrected at inspection The maintance and construction men are here working in the building to fix the tiles in the ceiling.

8 out 10 staff did not have training in emergency prepardness.
