Caregiver in charge of a toddler's class failed to supervise a 20-month-old child who fell from a play structure. The toddler was observed on the ground for about 15 seconds before a caregiver noticed.
During walkthrough 4 years old classroom was observed having screen time while eating lunch. Deficiency was corrected during inspection. I explained caregiver standards about screen time. Caregiver was asked to turn Television off.
During walkthrough 12 infants ages 3-11 months were observed in room 8. Corrected during inspection. Director was asked to use 3rd caregiver present to move 2 infants out of room 8.
Caregiver was observed inappropriately grabbing children by arm, leg and face. Caregiver was observed throwing a child’s lunch away before the child finished eating.

A toddler was observed sleeping on a stack of 7 cots. Deficiency was corrected during inspection when inspector asked caregivers to place toddler on one cot.
A child was not buckled while in a buggy and fell out.
Caregiver without First Aid Certification was left alone with children.
Caregiver in a 12-17 months infant room was out of ratio. Caregiver was alone with 8 infants.
An infant was not properly supervised while seated in a buggy. Caregiver was involved in a conversation with another staff and was not able to intervene when the infant fell out of the buggy and was injured.
