One of the ten children files did not have updated immunization records available for review.

Based upon information gathered and interviews conducted supervision was determined to have occurred when a child was left alone in the classroom sleeping.

Based upon information gathered as a result of this investigation, documentation and interviews conducted it was determined that the operation failed to notify Licensing timely of a child left in the classroom unsupervised.

The operation did not report the number of employees who ceased employment in 2023 by the required date 1.15.24.

Five of the ten child files were missing physician/parent signatures, and/or action plans for diagnosed allergies/medication.

The last annual fire inspection posted was dated on 10 5.2022.

Medication stored in the nurses office was expire on 4/2022.

Based upon information gathered and interviews conducted it was determined two children were left at the Zoo's tiny tots play area unsupervised by the daycare staff.
