A child had an allergic reaction while in care at the operation on 04/11/2023. This reaction resulted in the child needing and receiving medical attention.

There were caregivers taking care of toddlers and infants that did not have training for "Recognizing and preventing shaken baby syndrome and abusive head trauma, understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS), and "Understanding early childhood brain development"

There was not a written activity plan up to date in any of the classroom.

There were child seen climbing and not having their feet on the floor and caregivers were not redirecting the children in any manner. There were also children seen jumping on their cots.

Caregiver at the operation did not display good judgment or self-control when inappropriately grabbing children. Staff at the operation did not use good judgment in not reporting concerning behaviors of a caregiver to the director.

Incident report received did not contain at least the same information as Form 7239.
