During the walk through of the operation the Annual Fire inspection appeared to be expired.

During the record keeping check the director appeared to no have an updated CPR/Fiirst aid card.

During the walkthrough of the outside area the use zone for the slide is less than 6 feet.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The director does not have current transportation training.

A fire drill has not been conducted since May of 2024.

The affidavit in one file has not been notarized.

The ceiling tile above the sink in the pre-k room is missing. Paint is also peeling off of the wall. School age room has paint peeling.

The director's credential expired on 7/26/2024.

One caregiver was caring for 7 toddlers, 1 infant, and 4 three year, 1 four year old and 1 five year old, for a total of 13 children. The oldest child was more than 18 months older than the youngest infant.

The ceiling tiles were missing or stain in the following classroom indoor play, schoolers boy's restroom, infants and the kitchen. In the indoor play area also had chipping paint along the wall.
