This standard was evaluated based on evidence gathered from a DFPS investigation. During the course of the investigation, it was determined that the operation failed to submit a background check for a caregiver at time of hire. Records show that the caregiver's background check has been inactive since 4/29/2023.
Caregiver did not provide a sufficient walk and work space for caregivers between each cot and mat.
Infant was observed asleep in a restrictive device (rocker).
