The total number of employees who had left the operation in the previous calendar year was not completed by the January 15 as required.

Director changed three weeks ago and it was not notify to licensing withing five days after the change.

The person in charge during the director absence, was unable to have complete access to records requested and authority to direct the child-care center in compliance with minimum standards.

At the time of inspection the operation's last CPSC review was documented as 2/1/21.

At the time of inspection, the operation had not had an annual health inspection conducted.

The operation did not report concern of a child being placed at risk to licensing in a timely manner.

Documentation of fire drills being conducted monthly was not available for review during the inspection.

There is no section in the parent handbook or form for the parents to fill out if they are bringing meals from home stating they are responsible for the nutritional value of the meals they are providing. There is currently one child that brings meals from home.

Documentation of lock down drills being conducted was not available for review during the inspection.

The operation does not complete a daily building and grounds checklist.

Documentation of severe weather drills being conducted was not available for review during the inspection.

A caregiver did not practice good judgment or self control when they were focused more on the negative aspects of children's behavior when communicating with parents.

A caregiver threatened their own child with corporal punishment while in care.

There are multiple concerns of a caregiver grabbing children in a harsh manner.

A child touched another child on a private part on the playground and none of the caregivers saw or heard this incident happening.

The child sustained a head injury on the playground, which operation staff was unaware of or did not respond appropriately to.

A child was not provided first aid following a head injury on the playground.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance. The operation did not report to CCR within two days after becoming aware of the positive COVID-19 cases.
