Based on interviews with the staff, the parent of the victim, a collateral, and the victim, a child was left outside on playground without an adult present.

This standard was evaluated as part of the Health and Safety Audit and was found be deficient. The last fire drill conducted by the operation was 12/11/2024.

The operation was not current on practicing their monthly fire drills. This was corrected at inspection when the operation conducted a fire drill.

The operation does not have a fire inspection conducted by a fire marshal.

Based on the interviews with staff and the parent, it is determined that the caregiver did not use good judgement to escort a young child which resulted in the child having a dislocated arm.

Three infants were placed in seats at a infant/toddler tabled that have safety straps were that were not securely fasten. The deficiency was corrected during the inspection when the director securely fasten the safety straps around each child.

Evacuation and relocation diagrams were not placed in the gym, an infant, and prek classroom. The deficiency was corrected during the inspection when the management placed the diagrams in each activity area.

The outlets in the gym where children were playing and two outlets in one of the four year old classrooms was not covered. This was corrected during the inspection.
