Four caregivers are missing 3 hours of the required 24 annual training hours.

The director is missing 6 hours of the required 30 annual training hours.

The play structure's do not have a use zone of six feet. On one play structure it is only 3 ft and 9 inches from the barrier. On the other play structure they is a play house in front of the slide.

The director is missing 20 hours of annual training,

Two children did not have the Epi-Pens in the original container they were placed in a zip lock bag.

The operational policy did not have parent rights.

The operational policy did not have the correct information on where to find Minimum standards or HHSC website.

One child that is four years old did not have a vision and hearing screening on file.

During the walkthrough of the playground the operation was found to have failed to maintain 6 inches of loose filled material for equipment less than 5 ft.

During naptime the operation was found to not have adquate space for caregivers to walk between naptime mats.

During the review of the operation's emergency preparedness plan, the operation failed to provide a plan for how children with limited mobility will be evacuated/relocated and the operation did not provide an addess for an alternate shelter for relocation as required by minimum standards.

Employees who Left Employment in 2021 survey was not completed by deadline of 2/22/2022.

Six staff records were reviewed. Three caregivers are missing proof of education from their personnel file.
