A child was observed walking into the office without supervision and caregivers were unaware he had walked away.. In another incident children were observed transitioning inside without proper supervision, as a caregiver was observed walking to get a child who had walked off, leaving the children in the middle of the line were unsupervised. The children in the end of the line were supervised by a second caregiver, however she could not see the children in the middle of the line.
A child was observed to have walked out of a classroom and walk outside with another group without a caregiver supervising the child during the transition.

There was no caregiver observed in a 4 year old classroom during nap time.
A caregiver was observed putting her hand on a child's head and telling the child to go back to sleep. Naptime had started at 12:00pm and the caregiver was observed after 1:00pm telling the child to go to sleep.
A child was observed placed in a classroom with no other caregiver present and the caregiver who placed the child in there walked out and closed the door.
