Three caregivers had completed sexual abuse training. Training did not include neglect training. Note: This is a recite.

The operation was not able to produce documentation that daily playground checks were conducted to check the depth of the use fill surface and that the swings on the composite structure had been removed. There was a bucket swing and regular swing attached to the composite structure at the time of the inspection which were removed.

Three out of three staff files reviewed did not contain documentation of a completed Abuse/Neglect training for the evaluation year 8/2025-7/2026

A caregiver with a hire date of 3/2025 did not have documentation of completing pre-service training.

The operation moved a prek classroom into a classroom that was previously used for their private school program. This room was not previously inspected or measured for the use of the prek children. NOTE: Photos and measurements were taken during the inspection and added to the operation's space sheet.

The operation did not have a current fire inspection report on file.

A caregiver with conditions listed on their eligibility letter was observed supervising children alone. NOTE: The caregiver combined with another classroom and eligible caregiver during the inspection.

Children in the Prek classroom were observed eating snack on the carpet in the classroom and children were pouring their snack onto the rug and eating off the rug. NOTE: Children were moved to a table in the classroom.

Upon reviewing child files, a child who was under 3 years of age was enrolled at the operation. The operation's current permit is for Prek children, ages 3-4 years old.

The 3 year old classroom needed a piece of the tile floor installed that was missing and the molding in the center of the room and on the right side of the classroom had not been installed. This is a re-cite.

A fire inspection report was not available for inspection.

The small climber designated for use by the operation has a swing and bucket swing attached to the climbing unit.

Written approval from the Fire Marshal had not been obtained to care for children on the second floor.

The 3 year old classroom had construction being completed for drywall and painting

An initial health inspection was not available during the inspection.
