There was a child's EpiPen on site that expired in September of 2023.

Five out of the eight staff files reviewed did not meet the required 24 hours of training, and all eight were missing trainings in the required topics.

Documentation of smoke detector, carbon monoxide, and fire extinguisher checks was not available. As well as the playground maintenance checklist.

There is no documentation of a current sanitation inspection on file.

An active background checks was not available to review for one caregiver who was present during the inspection.

Current fire inspection was not available for review during the monitoring inspection.

The operation's last sanitation inspection was on 6/8/2020.
