There were dirty dishes and syrofoam beverage drink in the handwashing sink.

Eight children had expired medication.

Three caregivers did not have HASA trainings completed.

While reviewing the operation policy there was no promotion, benefits or duration of physical activity available.

There was no documentation to prove that a new staff received orientation prior to being placed in ratio with access to children. *This was corrected at inspection when orientation was finalized and the documentation was updated.

One caregiver was placed in ratio of a classroom before meeting the requirements for preservice training. The staff had less than a month of prior experience in regulated child care.

There was no proof of High School Diploma/GED to verify that a new staff meets the education requirements for caregiver qualifications.

There was no notarized affidavit on file for a new staff present and accessible to children.

During the course of the investigation it was found that caregivers were present at the operation inside classrooms when they did not have cleared fingerprints in the system. Caregivers were sent home and informed they could not return until eligibility has been determined.
