It was found through investigation that the operation did not maintain the required number of caregivers for a field trip to a public space based on the ages of children present.
Based on information obtained through investigation it was found a caregiver grabbed a child by the arm and pulled them up off the ground when they did not want to go to use the restroom.
At the time of the inspection 1 of the 10 records reviewed did not have an immunization record.
At the time of the inspection 2 of the employee staff files did not have proof of education requirements.
