The primary large playground do not have proper depth of mulch maintained throughout the area. .
A classroom was also observed to have a plug outlet without a cover. ** The caregiver placed covers on the plug outlets. **
2 classrooms were observed to have diaper rash cream in unlocked drawers accessible to children. ** The caregivers removed the cream and placed it out of children's reach. **
A caregiver grabbed a child by the chin and caused his neck to jerk backwards. The same caregiver was also observed grabbing the child by the arm causing the child's entire body to jerk back and forth.
The director did not demonstrate good judgment when allowing a caregiver to remain in the classroom after observing that caregiver inappropriately redirect a child.
An infant had a stuffed blue bunny animal on a crib mattress while sleeping. The infant was sleeping on top of the stuffed animal and was seen when the infant moved.
During inspection one infant was sleeping in crib with two mattresses stacked on top of each other. Crib manufacturer instructions state that only 1 three inch mattress is to be used in the crib. Note: This was corrected by removing additional mattresses inside cribs.
