Child was observed going to the restroom. Restroom has a jack and jill set up, connecting two classrooms. Child walked into an empty classroom, when exiting the restroom. Caregiver was not aware child had gone out the other exit of the restroom.

A four year old child was left inside the classroom’s restroom, unsupervised, when the class left to the outside playground.

Electronic smoke alarms were last inspected on 03/11/2021.
