A child walked out of the classroom into the hallway unsupervised.

There were two separate incidents in which a caregiver stepped out of the infant room leaving the infant room out of ratio.

A caregiver in the infant room dozed off in the rocking chair while holding an infant during feeding.

A child went to the restroom and wondered into the next classroom over. There was no caregiver in the next classroom which left the child unsupervised.

A caregiver was observed on their tablet while they were suppose to be supervising children.

One infant was observed in the crib with a pacifier strap attached to the pacifier.

School age Caregiver, was unaware how many children she had in care.

Supervision was inadequate when the caregiver in the after school room left a child unsupervised in the restroom and another child unsupervised in the classroom.

This standard was reviewed in relation to the allegation and found to be deficient. Caregivers did not supervise the children appropriately when leaving the classroom without aa child and without counting the children.

Based on the caregiver's interviews the caregiver did not redirect the children in a positive manner.

Two out of eight infant feeding schedules were not updated in the past month.
