An infant in care did not have updated feeding instructions on file. The instructions on file were dated 11/25/2025. The parent updated and signed the feeding instruction form during the inspection.

One out of ten updated immunization records were not available at time of inspection.

Two out of four infant bottles were observed not labeled with first name and last initial. Corrected at inspection when all bottles were labeled. All infant caregivers were retrained on labeling bottles at time of inspection.

Nap time room was over ratio by 5 children. Three children were ages 12-17 months old. This was corrected at time of inspection when another caregiver was added to ratio.

An allegation of prohibited punishment of a child was not reported.

A caregiver on occasion would yell at children when frustrated.

A caregiver did not demonstrate self-control by pushing a child after the child tried taking a toy from another child.

Two caregivers did not have current CPR training to review.

One EpiPen for a child expired was expired.
