Electronic child safety alarm in vehicle is not working. Transportation van seats 15 passangers.

Loose fill surfacing in use zone of dixie dozer in oustide playground, had a depth of approximately one inch.

The use zone of a 5 seat merry go cycle overlapped with the use zone of a stationary bus. There were approximately 10 feet between playground equipment. Stationary bus had approximately 2 feet of use zone on the side facing the wall.

Six children's files were reviewed. Four files were pending updated immunization records.

The fire inspection was last conducted on 3/28/2023.

Two out of six employee records evaluated did not have an affidavit on file.

The health inspection was last conducted on 3/1/2023.

The fire extinguishers have not been serviced since October 2022. (Fire extinguishers inside of the operation and in the transportation vehicle)

A caregiver without an active background check was observed to be supervising children.

Two caregivers were observed to be supervising 12 infants. 2m, 4m, 6m, 9m, 10m, 10m, 10m, 11m, 12m, 12m, 12m, 16m. SAG 0m to 11m. Room was out of ratio by 2 infants. NOTE: This was corrected at the time of inspection. One infant left and another infant was moved to another class.

A ball was observed to be inside the play yard while a 10 month old infant was sleeping.

One employee was missing the affidavit.

The vehicle used for transportaton does not have an electronic child safety alarm.

Four out of six employees were missing the hands-on CPR training. Training was done online. One out of six employees was missing the CPR/First Aid training.

Five out of six employees are missing trainings in the follow topics: hazardous materials, allergic reaction, administering medication, emergency preparedness, building safety communicable diseases, and transportation.
