Upon review of center personnel files, 2 of 5 files reviewed lacked the documentation for caregiver education.
During the walkthrough a caregiver was observed during the diaper changing process. At the completion, the caregiver failed to properly clean the childs hands as well as her own hands.
Upon review of personnel records, a caregiver did not have up to date first aid & cpr training.
During the inspection, a trashcan was observed without a lid in one of the classrooms. The playground was observed with trash on it.
During the inspection there was a person present with direct access to the children without an active background check. The person was being counted into ratio. This was corrected at the inspection when the person was relieved by another caregiver and left the center.
During the inspection a caregiver was observed changing a child's diaper but did not wash their hands or the child's hands afterwards.
