Based on evidence obtained during the investigation, it was determined that a caregiver used inappropriate discipline as the caregiver grabbed a child by the arm, leaving fingernail indentations.

Playground equipment has not been adequately maintained, creating a risk for children when utilized. Loose boards and gaps were observed on a play structure used by young children.
