During the inspection, a caregiver in a toddler classroom was found to not have an active background check with the operation. The Director had the caregiver leave the operation immediately until a new background check could be requested and cleared.
Four of the eight infants in care were missing current feeding schedules.

Based on the information obtained during the investigation by DFPS, it was determined that the caregiver was aggressive and didn't use control when a child was pulled, yanked, and forced back onto their mat to sleep.
There was a child left unsupervised by day care staff at an in-house field trip for an amount of time.
A caregiver that was responsible for supervising, did not have pediatric CPR training at the time of the incident.
