A review of the Employees Who Left Employment in 2022 data from February3 ,2023 revealed that the center had not reported the number of employees who left employment in 2022 as required.
The director did not have her current first aid and CPR training on file.
There was no photo identification on file for one out of five records checked. This was corrected during inspection.
