CCR observed a caregiver park the van used for transportation without completing a walk-thorugh and visual check of the vehicle as well as not disarmining the alarm from the rear of the van.
During the inspection I observed a caregiver not wash their hands in between cleaning two infants noses.
On the playground I observed 5 exposed screws on wooden boards as well as an active playground. This was corrected when the director placed treatment on the ant pile and the screws were removed.
During the inspection 1 caregiver was caring for 13 children with a specified age group of 2 years old.. Ratio is 1-11. The caregiver was over ratio by 2. This was corrected when the second caregiver went back into the room.
During the inspection I observed a school age child walk out of the classroom into the halllway as well as a 4 year old go to the restroom unsupervised.
During the walkthrough of the building I observed a mop bucket with water in the hallway accessible to children in care. This was corrected when a caregiver moved the bucket out of reach of the children in care.
During the inspection I observed a box of prescription medication sitting on a rolling cart accessible to children in care. This was corrected when the caregiver removed the medication from the room.
During review of naptime a room was observed to not have adequate lighting during naptime. This was corrected when the caregiver opened the classroom door to allow more lighting.
During the walkthrough of the infant room the cribs were found to not have labels as required. This was corrected when the director printed and placed lables on the cribs.
During review of infants 3 infants not yet eating table food did not have current feeding instructions as required.
During review of infants room the cribs were found to be close enough for infants to reach into another crib. This was corrected when the director rearranged the cribs.
During the walkthrough of the playground, I observed a seesaw without surfacing. This was corrected when the operation removed the play equipment.
During the inspection the operation was found to not have provided notification to parents that they no longer had liability insurance by the 3/1/2025 anniversary date. This was corrected when the director completed and posted form 2962.
During the inspection I observed a caregiver without cleared eligible results caring for 6 children between 20 months and 2 years old. Correction: Director sent caregiver home. Will not return until results have been received.
During the walkthrough of the playground, I observed 3 broken fence boards with sharp edges.
