One classroom was observed not having the current activity plan posted in the Toddler Classroom.
Annual training was not complete for the Director. Director has completed 15 hours of annual training for the 20-21 training year.
Infant Daily reports were not complete for six infants that were in care at the time of inspection.
Two children's records reviewed did not have the completed designated contact information on the admisison form at the time of inspection.
Two personnel records reviewed did not have the required 24 hours of annual training complete for their annual training year.
Operation allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility.
Annual Fire Inspection has not been completed for the year.
