During inspection it was observed that 13 cribs had additional mats in the cirbs. One of the 13 cribs had two full size mats and a thin mat. One of 13 cribs had two full size mats in the cribs.

Two caregiver's did not have a background check submitted. Note: One staff member was counted in ratio during walk through and left operation at 1:00 pm and other caregiver was not present during inspection. Background checks were submitted during inspection.

A newly added playground did not contain 2 exits.

There was one gate on the 18+ playground that did not close at the ground level creating an approximate 2 in gap. On the back playground where the fence material did not meet the shed, there was a large enough gap where children could exit the playground.

Former caregiver was seen in video footage grabbing and pulling a child across the carpet.

Former caregiver was seen in video footage hitting a child in the foot with the child's shoe.

Five of the ten personnel records reviewed did not have the employment affidavit in file.
