The fire extinguisher used in the transportation vehicle was not secured in the passenger area. The fire extinguisher was last serviced in April 2023.

There were 6 ceiling tiles with water stains in the operation. There were two vents in a classroom with dust and debris.

There was a tire swing in the outdoor area without surfacing material. Corrected at Inspection: The swing was removed.

A caregiver had 4 out of 24 hours of annual training topics for training period, 4/10/23-4/10/24.

There was one electrical outlet without a cover in the Toddler classroom. Corrected at Inspection: The electrical outlet was covered.
