During the course of the investigation, it was determined that a child was left unsupervised on the playground for 20 minutes.
One of ten staff records reviewed for CPR training did not consist of a curriculum that included hands-on skill-based instruction.
A fire inspection was not completed every twelve months. The last fire inspection was conducted on 9/4/2024.
The caregiver in the preschool classroom gave the incorrect number of children in the classroom. Caregiver stated there were fifteen children when there were sixteen children in the classroom. Note: This was corrected at inspection when caregiver recounted the children and provided correct number of children in their care.
During the course of the investigation, it was determined that caregivers did not follow transition protocol which resulted in a child being left behind in a classroom unsupervised.
The activity the children were participating in did not match what was posted on the written activity plan. Correction was made by adding the activity to the activity written plan.
