A caregiver was missing 12 hours of annual training for her evaluation period. The caregiver only had 12 hours of training available for review.

A caregiver was missing shaken baby/SIDS/brain development training. The training was not available for review. The caregiver was observed supervising children under 24 months during inspection.

The operation does not have a current sanitation inspection. The last sanitation inspection was conducted on 3/10/25.

The operation's fire extinguisher for the building has not been serviced. The last time the fire extinguisher was serviced was January of 2024.

The operation did not conduct a fire drill for the month of June 2025. No documentation of the fire drill was available for review. This was corrected at inspection after a fire drill was successfully conducted to make the operation come into compliance.

Director did not have current training in pediatric first aid with rescue breathing and choking. His/her training expired on January 2025.

A transportation bus used by the operation and required to have an electronic child safety alarm system did not have a working alarm.

The operation does not have a current gas leak inspection. The last gas inspection was conducted on 11/22/21.

Director is missing 6 hours of annual training for evaluation period 10/2/22 to 10/2/23. Director only has 24 hours of training from the required 30 hours.

Director and one caregiver did not have current transportation training. The last training was taken in January of 2022.

Reviewed operation’s people list CLASS on 02/02/2023 and observed director still ACTIVE as an employee, the operation failed to inactive director. Director last day of employment at the operation was on 10/22/2022.

Three out of five employee files checked were missing 10 hours of required annual training hours. ---------------- Three out of of five employee files checked will need to complete the following required training topics: Emergency Preparedeness, Preventing Communicable Diseases, Medication Administration, Food Allergies, Physical Safety, and Handling hazardous materials.

One out of six children's files checked did not have a Health Statement on file for review.

One out of six children's files checked did not have an immunization record available for review in their file.
