Observed the fire drill had not been documented as being completed in October. This was corrected when the operation completed a fire drill in 1 min. 52 seconds.

A report was made, but not properly, and did not get reported within the required 48 hours. This was corrected when the abuse and neglect hotline number was given to the director and the proper procedures for reporting were went over with the director.

A child was left outside for around 10 seconds without a caregiver. This was corrected when the caregiver noticed the child was not in the group and went and got the child from outside.

The operation failed for report positive covid cases within the 48 hours required time frame. This was corrected when the director was given required timeframe and how to report any new cases.

Observed two caregivers who work with children under 24 months of age with training on shaking baby syndrom, SIDS and early childhood brain development.

Observed 3 infants sleeping in the cribs with the side railing being down. This was corrected when the drop down railings were raised and locked.

Observed a caregiver in the hallway and when licensing went into the room with the caregiver, there was no other caregiver in the room. There were children in the room by themselves while the caregiver was in the hallway.
